By completing the application form, you are hereby applying to be an Independent associate and member of THE Golf & Travel Club and agree to all of the terms and conditions set forth in this Application. You acknowledge that other than the Membership and Business Start Kit for $79.95, you do not have to purchase any other products. You agree that you are not purchasing a product, but are purchasing the Membership and Business Startup Kit ,which includes the following:

• Membership to THE Golf & Travel Club. This includes a one (1) year membership, an Associate ID, the rights to market and promote the Company products, and the opportunity to participate in the Associate Program

• The Rules and Regulations of the Company (electronic format).- The Business Development Manual (electronic format).

• Setup of an Associate Website and the first month hosting and maintenance fees. After the first month, the monthly hosting and maintenance fee is as shown. The Associate will select the option that works best for them.
            - $30 per month paid monthly, or
            - $81 per quarter (save $3 per month), or
            - $144 for 6 months (save $6 per month), or
            - $240 for 1 year (save $10 per month)

• Access to "The Clubhouse", the Associate back office.

The Membership and Business Startup Kit is required because only members of THE Golf & Travel Club can market and sell the Company products, as per our agreements with many of our suppliers. The cost of this kit is fully refundable if an Associate cancels their application within the prescribed allowable period (72 hours after purchase). There will be NO refunds after 72 hours.

By purchasing the Membership and Business Startup Kit, you agree to the following terms:

• You are running your own business as an independent member of THE Golf & Travel Club.

• You are not employed by the company as an employee or contractor. You are selling THE Golf & Travel Club products directly to consumers and are purchasing them at wholesale from THE Golf & Travel Club. This includes both the products and the memberships.

• When eligible for profits on the sale of products, you are paid directly by customer and not by THE Golf & Travel Club.

• As an independent member, you have access to THE Golf & Travel Club business resources to help your business provided your membership is in good standing. There may be restrictions on some of the resources as some of them are only available to our Stage Members. These resources remain the property of THE Golf & Travel Club. THE Golf & Travel Club reserves the right, in its sole discretion, to modify, alter or otherwise update these resources at any time and by using the resources, you agree to be bound by such modifications, alterations or updates.

NOTE: THE Golf & Travel Club retains the right to reject an application for whatever reason. If THE Golf & Travel Club rejects an application, the purchase price will be refunded.

You may cancel this transaction, without penalty, or obligation, at any time within three (3) business days from the date of this application.

If you elect to cancel this transaction, you will be required to return any products back to THE Golf & Travel Club in substantially the same condition as when you received them, as well as any other goods and products delivered to you under this Application and Agreement. If you fail to return the goods and products to THE Golf & Travel Club, then you shall remain liable and responsible for the full amount and faithful performance of all obligations under this Application and Agreement, as well as any other agreement relevant to the transactions between you and THE Golf & Travel Club.

Applicant hereby applies to be an independent THE Golf & Travel Club Associate and agrees to all of the terms and conditions set forth in this Application. Applicant acknowledges that an Associate is not an employee, consultant, or investor in the company, but an independent business with the authorization to market and promote THE Golf & Travel Club products.

Before submitting your application, you must check the box stating that you have read the Terms and Conditions and agree to them. As well, you must type your name in again and the date to show that you have done so.
